All forms of identification must be current and un-expired. You must present one of the following credentials to your notary during the signing appointment: Unexpired Driver License or Non-Operating ID Card issued by a U.S. state or territory. Unexpired US Passport. Unexpired Armed Forces Identification Card issued by any branch of the US Armed Forces. Inmate Identification Card issued Arizona Department of Corrections, Federal Bureau of Prisons or a County Sherriff's Department within Arizona. (MUST BE CURRENTLY IN CUSTODY IF PRESENTING THIS TYPE OF ID) Other Unexpired Government-Issued ID Cards (Must contain the signer's signature, photo, and a physical description that includes height, weight, eye color and hair color.) Unexpired Foreign Government-Issued Consular Identification Cards (Must contain signer's signature, photo, date of birth, and dates of issue and expiration. Issuing agency must also use biometric ID verification techniques including fingerprint and retina scans.) IF THE SIGNER CANNOT PRODUCE ANY OF THESE FORMS OF ID: A credible witness may be used to identify the signer. The credible witness must be present during the signing. The credible witness must personally know the signer and must present one of the acceptable forms of ID listed above. The credible witness must also be placed under oath to affirm the signer's identity.
